What paperwork is required for admission?

A signed agreement between the resident or responsible party and the nursing home representative covering medical care, charges and refund policy.

What information is needed from the primary care physician?

A physical examination or current medical report prior to or upon admission, including:

  • Current medical findings and diagnosis;
  • Chest x-ray report and Mantoux (tuberculin) results;
  • Evaluation of restorative potential and rehabilitative procedures;
  • Hospital treatment summary (if the resident was admitted from a hospital);
  • Medical and social plans; and Orders for continuing care.

What can I expect once admitted?

On your scheduled arrival date, you will be greeted and welcomed by our staff. We will spend some time that day helping you get acclimated, reviewing your plan of care, discussing any questions you may have.

What should I bring with me?

Our rooms are furnished with the basic necessities. You will need your own clothing and personal care items. Please ask our admissions team if you have specific questions on what you can bring.
